Comments:
Licensing Inspector arrived at 9:45am and departed at 2:00pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 52 children present, ages Infant through five years in care with nine staff members. Children were observed during circle time, playing in centers, coloring and during music and movement. Infants were observed having tummy time, sitting in bouncy seats and engaging with staff. Five children's records and Five staff records were reviewed on this date. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-004-F
Description: Based on observation and review, the vendor did not ensure that there was a current list of all children's allergies, sensitivities, and dietary restrictions. Evidence: The list of children's allergies was not current, as a child's record was reviewed, and a documented allergy was not included on the list.

Plan of Correction: The list will be updated today.

Standard #: VENDSUB-000-006-B-11
Description: Based on review, the vendor did not ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The center had not obtained an allergy care plan for a child with a diagnosed allergy to peaches and citrus fruit.

Plan of Correction: We will ask the father to make the appointment or give us permission to fax the form to the physician today.

Standard #: VENDSUB-000-006-B-3
Description: Based on record review, in four of five children's records reviewed, the vendor did not ensure that all required parent contact information was in each child's record. Evidence: There was no documentation of one or both parent's complete work addresses in the records for Child #1, Child #3, Child #4 and Child #5.

Plan of Correction: All parents will be contacted for the work addresses.

Standard #: VENDSUB-000-031-B-3
Description: Based on review, the vendor did not ensure that the center's written emergency preparedness plan addressed all required procedural components for lockdown. Evidence: The following components of the center's lockdown procedures had not been developed: 1. Methods to account for all children in the lockdown locations 2. Methods of communication with parents 3. Accommodations or special requirements for infants, toddlers, and children with special needs to ensue their safety during lockdown 4. Procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child

Plan of Correction: The plan will be updated today to include all components.
