Comments:
An unannounced monitoring inspection was conducted today March 20, 2018 to determine compliance with licensing standards. The inspector initiated the inspection at 11:00 am and concluded the inspection at 2:00 pm. The Director participated in the inspection. Upon the inspectors arrival children were finishing activities and preparing for lunch. Children washed their hands before sitting down for lunch which was served family style. Staff sat with children and interacted in a positive manner, conversing about the weekly theme of space. Children in the pre-school class told the inspector the name of the planets and facts about the planets. In the infant room children were observed playing on the floor with staff. Children played with soft puzzles, plastic animals and colorful blocks. Staff reviewed colors and sang songs. Some infants were eating. Staff sat with children and washed their face and hands after the meal. Children in the 2-3 year old class gathered as a group on the carpet to sing songs and participate in the songs with movement. Nap time was observed in the toddler class with soft music being played in the background and staff sitting next to children patting their backs. Staff interacts were positive. The theme throughout the center was "Space". Inflatable plants hung from the ceiling, books about space were read and children had make space helmets. All areas of the facility were inspected to include classrooms, kitchen, hallway and bathrooms. The playground was not inspected due to inclement wether. The center has age and stage appropriate materials and supplies. Classrooms have children's art work displayed along with photos of children participating in S.T.E.M. projects. The facility had the following required information posted: the License, last inspection notice, emergency numbers, evacuation maps/procedures, daily schedules, menu and various parent information. Today's lunch consisted of Italian ziti pasta with turkey sausage, broccoli, pears and milk. The first aid kit and non-medical emergency supplies were checked and found complete. The inspector interacted with staff and children in care. Nine staff records and eight children records were reviewed. Medications and medication authorizations were reviewed. Staff on site with MAT certification. Staff have current CPR & First aid training along with daily health observation training. The most recent evacuation drill was documented on 02/13/2018. The two required shelter-in-place drills were completed on 01/31/2018 and 02/13/2018. Standard #: 22VAC40-185-160-A
Description: Based on review of staff records, the facility did not obtain documentation of a tuberculosis screening for staff that had been completed within 12 months prior to hire or 21 days after employment. Evidence: Staff #6, hired 12/11/2017 had documentation of a tuberculosis screening dated 01/10/2018, exceeding the 21 day from hire allowance to obtain.

Plan of Correction: Per the Director: A system is in place to prevent any employee to start without TB screening completed.
