Comments:
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 9:00 AM and departed at 12:15 PM. Fifty-nine children were in care with nine staff. Morning activities and breakfast service were observed. A sample size of six children's records were reviewed. Medication administration was reviewed. An exit meeting was conducted with a program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based upon observation, the facility has not ensured that areas inside and outside are maintained to be safe. Evidence: 1. One fence panel of a wooden enclosure around an air conditioning/heat pump unit on the playground area has come off, allowing access to the air conditioning unit. 2. One fence picket on a section of the wooden enclosure around an air conditioning/heat pump unit on the playground has come off and is on the ground. A nail is exposed on the fence piece.

Plan of Correction: The facility responded with the following: Maintenance will be notified to repair the fencing.

Standard #: 22VAC40-185-510-A
Description: Based upon review of medication and staff interview, the facility has not ensured that medication is given only according to the center's written medication policy. Evidence: 1. There was an epi-pen jr. for child 1 that expired in December 2017. Staff 1 acknowledged that the epi-pen jr. expired in December 2017. 2. There was beclomethasone dipropionate for child 3. Although the parent signed an authorization form, the authorization form did not have indicated the medication or dosage of medication that the parent was authorizing to be administered. Staff 3 acknowledged that the authorization to administer the medication form did not have indicated the name or dosage of the medication to be administered.

Plan of Correction: The facility responded with the following: All medication will be checked to ensure that all medications, required authorization forms, expiration dates, are obtained and tracked to ensure with facility policies and licensing regulations.

Standard #: 22VAC40-185-510-G
Description: Based upon review of the children's medication and staff interview, the facility has not ensured that prescription medication is dispensed from a pharmacy and maintained in the original, labeled container. Evidence: 1. There were 2 epi-pen jr. pens stored for child 1. The containers were not in an original, labeled container from a pharmacy. 2. Staff 1 was unable to provide a pharmacy container and label for the epi-pen jr. pens.

Plan of Correction: The facility responded with the following: All medication will be required to be in an original, labeled container from a pharmacy.

Standard #: 22VAC40-185-510-N
Description: Based upon review of children's medication and staff interview, the facility has not ensured that they notify the parent to pick up medication that is no longer being administered and, if not picked up, they dispose of the medication. Evidence: 1. There was an albuterol inhaler labeled for child 6. The albuterol expired in September 2017. 2. Staff 2 stated that child 6 has not been enrolled/attended this school year.

Plan of Correction: The facility responded with the following: All children's medication will be checked monthly and returned to the parent when the authorization or the medication expires. At the end of the school year, all medication will be returned to the parents. When a child withdraws from care, any medication will be returned to the parent.
