An unannounced monitoring inspection was conducted from12:30 p.m. to 2:40 p.m. At the time of entrance the following adult to child ratios were observed: Classroom D: 16 children, ages 3 to 5 years old, with 2 staff (nap period) Classroom C: 17 children, ages 3 to 5 years old, with 1 staff (nap period) Classroom A: 14 children, ages 3 to 5 years old, with 1 staff (nap period) Classroom B: 16 children, ages 3 to 5 years old, with 1 staff (nap period) The records sample size consisted of 8 children?s records. Staff records are housed off site and will be reviewed at a separate inspection. Children were observed resting quietly on cots during rest period and being dismissed / picked up by parents from the program. Hand washing and restroom procedures were also observed. Medication administration was not observed. However, 1 mediations kept on site for emergency use was reviewed with regard to storage, labeling, written authorization and required documentation. Staff report no accidents or injuries requiring first aid or medical care since the last inspection. Deficiencies observed during the inspection are noted on the violation report. Standard #: 22VAC40-185-510-C Description: The licensee did not ensure that the center proceeded with methods to prevent use of outdated medication. Evidence: Located in a locked medicine cabinet was a Ventolin inhaler for child #1 which had current accompanying paperwork attached to it authorizing administration of the medication as needed for breathing difficulties. The medication was labeled with an expiration date of January 2018. Plan of Correction: The program utilizes a "911" form authorizing the child to be treated by first responders in the event that emergency medication is needed but not available. This form was present for child #1 and will be utilized in an emergency until the expired medication is replaced. The expired medication will be returned to the parent today at pick up and the parent will be asked to bring in current medication to replace it.
