Comments:
Today an unannounced Monitoring Inspection took place between the hours of 9:25am and 12:00pm. At the start of the inspection there were a total of 60 children with 9 staff; all ratios were incompliance. A total of 5 children?s files and 4 staff files were reviewed. The children were observed singing songs for an assembly, having music class, eating their morning snack (everything crackers with hummus) and playing in small groups. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information: Evidence: Child #1 file did not contain documentation of the child's first date of attendance.

Plan of Correction: I'll fill it in. This was corrected during the inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances and cleaning materials were not kept in a locked place. Evidence: 1. Various bleach and cleaning materials are stored in a cabinet in the Penguins classroom. There is no lock on the cabinet. 2. There is a can of air freshener stored in the cabinet in the bathroom shared between the Dolphins and Tigers classrooms. There is no lock on the cabinet.

Plan of Correction: I'll go take them out now.

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning and sanitizing materials were not stored separate from food. Evidence: In the cabinet of the Penguins classroom, rubbing alcohol and hand soap was stored next to oatmeal and various baking spices.

Plan of Correction: They will fix it at nap time.

Standard #: 22VAC40-185-350-E-3
Description: Based on observation, the ratio of one staff member for every eight children was not followed. Evidence: When the inspector entered the Tadpole classroom, there were ten children with one teacher. The teacher confirmed that the children were between 2 and 3 years old.

Plan of Correction: We will make sure that there is coverage.

Standard #: 22VAC40-185-510-N
Description: Based on observation, medication was not returned to parents within 14 days of an authorization for medication administration expiring. Evidence: Child #6 medication authorization for an emergency medication had expired on 2/7/2018. The medication had not been returned to the parents.

Plan of Correction: We will give it to the parents tonight.
