- Technical Assistance:
Discussed with center management the need to assess center furnishings and playground area for wear and tear. Also discussed assessing the playground for any safety hazards before going outside.
An unannounced monitoring inspection was conducted from 11am to 2:15pm. There were 19 children in direct care of 7 staff members. Children were observed having lunch, getting diapers changed, using the bathroom and preparing for nap time. The lunch menu included pizza, mixed veggies, diced mangoes and milk. Child Supervision Records were reviewed alongside emergency drill records. 5 staff / children records were reviewed. The playground area was inspected as well. Standard #: 22VAC40-185-270-A Description: Based on observations, areas outside the center were not maintained in a safe and operable condition. Evidence: 1. The pour in place playground surface under the play equipment has a large hole causing a trip hazard for the children. 2. The plastic seat on a bike on the playground was broken causing an injury hazard for children. 3. The window frame outside the School Age classroom has a metal screen that has slid from it's original position, exposing the metal edges within the window frame. Plan of Correction: A work order will be placed as soon as possible and the bike will be removed from the playground.
Standard #: 22VAC40-191-60-B Description: Based on record review, center did not obtain a sworn disclosure statement (SDS) for an employee by the first day. Evidence: 1. Staff # 1 was hired on 10/3/17. 2. There was no written record of a complete SDS in Staff # 1's record. Plan of Correction: Staff completed the SDS during the inspection.
