The licensing inspector conducted announced monitoring inspections at the administrative building on Friday, March 23, 2018 to review staff records from 11:00AM until 2:15PM.. Standard #: 22VAC40-185-160-A Description: Based on review of six staff records and interview, the facility did not obtain a negative tuberculosis screening for one staff member. Evidence: During the review of staff records, the licensing inspector observed the record for Staff #2 (hire date 11/17/17) did not contain documentation of a negative tuberculosis screening. A member of management contacted the staff member during the inspection and Staff #2 stated he still needed to obtain the tuberculosis screening. Staff are required to submit a negative tuberculosis screening 12 months prior to or within the first 21 days of beginning employment. Plan of Correction: We will instruct the staff member to obtain the required information.

Evidence: During the review of staff records, the licensing inspector observed the record for Staff #2 (hire date 11/17/17) did not contain documentation of orientation. A member for management stated all staff had received orientation but the documentation form for Staff #2 still needed to be completed. All staff records are required to contain documentation of orientation training by the end of the first day of assuming job responsibilities. Plan of Correction: We will have the staff member complete the required form.
