Technical Assistance:
Discussed with center management to limit the number of children on the big playground until more clarification is obtained, reminded center management that out of state central registries are required for staff that has resided outside of VA in the last five years, discussed the use of more fitted sheets for cribs, positing of allergies.

Comments:
An unannounced monitoring inspection was conducted today from 10:35am to 2:15pm. There were 107 children in direct care of 22 staff. 5 staff and children's files were reviewed. The children were observed engaged in a variety of activities including but not limited to arts and crafts, listening to music, singing, playing with legos, engaging in Power tots gymnastic activities, eating lunch and playing on the playground. The lunch menu included shredded cheese, lettuce, tortillas, ground beef taco meat and milk. Emergency drill records and medications were reviewed. Areas of non compliance are discussed in the violation notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment in the center were not maintained in a safe and operable condition. Evidence: 1. In the toddler 1 room, the plastic sink for the home living area was broken, posing an injury hazard to the children. 2. In the toddler 1 room, peeling paint was found under the bulletin board.

Plan of Correction: The sink will be removed today. The paint will be fixed.

Standard #: 22VAC40-185-500-B
Description: Based on observation, staff did not clean and sanitize the diapering area after each diaper use. Evidence: 1. Staff # 2 changed a child's diaper on the diaper mat. Staff # 2 used an additional non absorbent mat to place under the child while changing the diaper. 2. The second diapering mat belonged to the child but it was not disposable. 3. Staff # 2 folded the mat after completing the diapering process and placed the mat in the child's diaper bin. 4. Staff # 2 did not clean and sanitize the second mat after it was used.

Plan of Correction: We will talk to staff about using the proper cleaning and sanitizing procedures.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, center did not obtain a central registry for staff within 30 days of employment. Evidence: 1. Staff # 1 was hired on 10/5/17. 2. Staff # 1's central registry was mailed in November 2017 and returned by the agency due to the form being incomplete. 3. Center did not mail a completed central registry as a follow up. The initial central registry was in Staff # 1's file but it was not complete.

Plan of Correction: We will mail the form today.
