Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on April 4, 2018 from approximately 10:10 am until 2:15 pm. Upon entrance there were 17 children and 3 staff present with children. Additional children and staff arrived during the inspection. The record review sample consisted of 5 children's records and 4 staff records. Children were doing stretches, exercises and eating snack. The infants were observed freely playing. The results of the inspection were reviewed and verified with the center director/owner. Standard #: VENDSUB-000-005-B
Description: Based on observation and interviews, the vendor did not ensure that for each group of children, the vendor maintained a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: During the inspection on April 4, 2018, there was not a hard copy of attendance with arrival and departure time for the two classrooms in the center.

Plan of Correction: The center director will create a form and start implementing it tomorrow to document when children leave and come in addtion to the sign in out done by the parents at the front door.

Standard #: VENDSUB-000-031-B-1
Description: Based on observation and interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an emergency to include evacuation, shelter-in-place and lockdown emergencies. Evidence: During the inspection on April 4, 2018, the emergency plan did not include reunification plans.

Plan of Correction: The center director will update the plan.
