Comments:
An unannounced renewal inspection was conducted today from 9:00am-11:30am. At the time of inspection, 40 children (infants through two years) were directly supervised by 10 staff and ratios were in compliance. The sample size consisted of 5 children records and 5 staff records. Children were observed participating in centers, having circle time, interacting positively with staff, and eating breakfast. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in medication administration, CPR and First Aid, as well as daily health observation training. Violations:
Standard #: 22VAC40-185-240-A
Description: Based on review of five staff records, the center did not have documentation that staff received orientation training that includes job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities. Evidence: Staff # 1 (Start date 9/17), was missing documentation of a completed orientation.

Plan of Correction: Director will have completed documentation in file by end of date.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center were not maintained in a safe condition. Evidence: In the Toddler room, a small area of chipping paint was observed in the back right corner.

Plan of Correction: Director will call the tech to set up an appointment for repair.

Standard #: 22VAC40-185-280-B
Description: Based on observation, potentially hazardous substances such as cleaning agents were stored in an area that was not inaccessible to children by a locking method. Evidence: In the preschool 1 classroom, 2 disinfectants and a glass cleaner were found in an unlocked diaper changing table. The locking mechanism was not working properly.

Plan of Correction: Director will contact the tech to set up an appointment for the repair.

Standard #: 22VAC40-185-510-C
Description: Based on observation, the center failed to prevent the use of out dated medication. Evidence: In the Infant room B, Child 1 was observed to have expired diaper cream.

Plan of Correction: Director removed the cream from the room and will return back to the parent.
