Comments:
The licensing inspector conducted an unannounced monitoring inspection Thursday, April 5, 2018 from approximately 10:15AM to 1:30PM. The following ratios were observed with a total of 122 children present during the inspection:
Satellite Infants - 3 children with 1 staff
Young Infants - 4 children with 1 staff
Older Infants -8 children with 2 staff
Toddlers (Red Room) - 13 children with 3 staff
Twos (Orange Room) - 16 children with 3 staff
Twos & Threes (Yellow Room) - 15 children with 3 staff
Threes (Green Room) - 11 children with 2 staff
Threes & Fours (Blue Room) - 16 children with 2 staff
Fours & Fives (Purple Room) - 16 children with 2 staff
Fours & Fives (Gold Room) - 18 children with 3 staff
Get Well ? 2 children with 1 nurse

During the inspection, children and staff were observed participating in a variety of activities. The Infants were observed playing on the floor with toys, having their diapers changed and eating lunch. The Toddlers were observed cleaning up from lunch, washing hands, looking at books and singing songs. The Twos were observed eating lunch. The Threes were observed playing outside, building with blocks and transitioning into lunch. The Pre-K children were observed playing outside, making frogs from paper plates, building with blocks and legos, and writing. Lunch consisted of spaghetti with marinara sauce, salad, tropical fruit and milk. Later children transitioned into nap time. All areas of the facility including classrooms, hallways, outdoor play area and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, daily attendance, daily sheet, practiced emergency drill logs, written allergy plans, menu, liability insurance and staff training information. Required postings were on display in the center. Medication is administered when required and medications and medication authorizations were reviewed.

During the inspection, 10 children's records were reviewed. Because the director was not present at the facility during the inspection, complete staff members? files were not reviewed. The emergency contact and related health concerns if applicable for staff were available and reviewed by the inspector. Complete staff records will be reviewed during the next inspection. Description: Based on observation, the facility did not ensure all electrical outlets contained protective covers. Evidence: During the inspection, the licensing inspector observed an uncovered electrical outlet located approximately one foot away from a crib in the Young Infant room.

Description: Based on observation, the facility did not ensure all prepared infant formula was labeled. Evidence: During the inspection, the licensing inspector observed three bottles located in the Satellite Infant's refrigerator that were not dated.
