22VAC40-191 Background Checks (22VAC40-191)
An unannounced Renewal inspection was conducted this date. A total of 63 children with 18 staff were on site. Observation of all six classrooms was made. Ratios were found to be in compliance. Hand washing, diapering and bottle feeding procedures were observed during the inspection. Staff interviews conducted to ascertain knowledge of the center's emergency procedures pertaining to fire evacuation and shelter in place drills. All medications reviewed and all topical ointments were reviewed. Transitions from morning programming to lunch was observed. Lunch today was chicken, tater tots, applesauce and green beans. A total of 5 children's records and 6 staff records were reviewed. This inspection began at 10:30am and ended at 3:15pm. Thank you for your cooperation today. Please email me at: email@example.com
Standard #: 22VAC40-185-440-G Description: Based on staff statements and observation of the Infant rooms, cribs were not used for children under 12 months of age. Evidence: Licensing Inspector observed one child, age 11 months sleeping on a cot located in the play area of the classroom. Staff stated that one other child sleeps on a cot. Plan of Correction: This was corrected immediately. All children under the age of 12 months will sleep in cribs.
Standard #: 22VAC40-185-520-A Description: Based on review of the topical ointments in the center, some ointments were kept and used beyond the expiration. Evidence: Aquaphor for Child #1 expired in 6/2008. Topical ointment for Child #2 expired in 6/2017 and topica ointment for child #3 expired in 1/17. Plan of Correction: All of these ointments were removed from the changing area.
Standard #: 22VAC40-185-520-C Description: Based on review of all diaper ointment maintained in the center, completed authorizations were not found. Evidence: Authorizations for diaper ointments were not found for Child #4, 5 and 6. Plan of Correction: We will obtain authorizations for these ointments. We will continue to remind staff to obtain completed authorizations.
