Comments:
An unannounced renewal inspection was conducted on April 10, 2018. The director and her assistant were present. There were 10 infants in care, all 12months and older, with 3 staff. In on of the Two Year Old groups there were 6 children with 1 staff, and in the second group there were 7 children with 2 staff. All of the groups were engaged in story time and letter activities. Later they washed their hands and ate lunch. Lunch for the day included chicken nuggets, peas, peaches, roll, and milk. Five children's records and 5 staff records were reviewed. No medication was being administered. Emergency drills had been documented. Inspection began: 10:20 Inspection ended: 12:30

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of 5 children's records, the center did not have documentation of a physical examination on file for each child who had been in care over 30 days. Evidence: The records on file for Child # 2, Child # 3, and Child # 5 (who had been in care over 30 days) did not include documentation of a physical exam.

Plan of Correction: The parents will be requested to provide the documentation.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center did not maintain the required resilient surface for fall zones on the playground. Evidence: The fall zones at the end of the 2 blue slides did not have the required resilient surface depth. The zone at the end of the curved slide had exposed dirt and gravel. The zone at the end of the straight slide had exposed dirt.

Plan of Correction: Mulch had been ordered to replenish the resilient surface at the fall zones.
