Technical Assistance:
Discussed with center management conducting playground inspections to avoid preventable injuries/accidents

Comments:
An unannounced non-mandated inspection was conducted in response to a complaint that was received by the licensing office on 3/22/18. Child/Staff records were reviewed. Injury reports were reviewed as part of the investigation. Interviews were conducted with staff relating to the allegations of failure to report an injury sustained by a child. The information gathered during this investigation support the allegations, so the complaint is determined to be valid. Description: Based on record review, the center did not notify the parent by the end of the day of minor injuries. Evidence: 1. On March 5, 2018 Child # 1 sustained a scratch under the right eye while playing on the playground. 2. The center did not notify the parent by the end of the day about the incident.

Plan of Correction: Parent will receive written report as well as phone call when incident /accident occurs. Management on duty on 3/5/18 maintains that she gave the child's caregiver the incident/accident report on 3/5/18. The caregiver told management that she did not want to sign the report but would give to mother. Management on duty that night then said that she called the mother the same night to let her know about the child's injury.

Description: Based on record review center did not maintain a written record of children's minor injuries on site in which entries are made the day of occurrence. When the injury report was written, center did not include, they were incomplete. Evidence: 1. The center did not have a written record of the injury Child # 1 sustained on 3/5/18 on site. Child # 1's file did not have a written record of the injury and center management could not find the copy of the injury report. 2. An incident report was written by Staff # 1 and provided to Child # 1's after care provider on 3/5/18. The written report was incomplete. The report did not include the accurate time of injury, future actions to prevent the recurrence of injury, and the documentation on how the parent was notified. 3. On 3/7/18, a follow up injury report was written for the injury that occurred on 3/5/18. The report did not have the correct date for "date/time should be when incident/accident occurred". The time when parent was notified was also missing on the report. This report was given to the parent on 3/8/18. 4. The injury report that was written on 3/19/18 for the second incident was not complete. This report had Staff # 2's signature but not of another staff or parent. The time that the parent was notified was not listed on this report.

Plan of Correction: All reports will remain on site and filed in proper place. All reports will be completely filled out accurately and on the date of incident. If the parent does not sign, 2 staff members must sign the incident report. Ab alternate form will be used to document follow up investigations of incident and accident reports. Staff will receive training by 4/20/18 on the proper way to fill out incident reports.
