Comments:
An unannounced subsidy Health and Safety Monitoring inspection was conducted from10am until 12noon. There were 12 children, ages 23months to five years old were in care at the time of arrival with three staff. Children?s and staff records were reviewed. Children were observed in caregiver and self-directed activities. Violations regarding staff training, one child's record and background checks were observed and appear on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to ensure that each child's record contains all required information. Evidence: 1 - The record for child 1 does not contain documentation of required annual update. The child has a documented first date of attendance as 3/6/17. 2 - Staff 1 confirmed that the record of child 1 past due for update.

Plan of Correction: Center management will ensure that each child's record contains all required information.

Standard #: 22VAC40-185-240-C
Description: Based on record review and interview the center failed to ensure that program directors and staff who work directly with children annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center. Evidence: The records for staff 2 - 6 do not contain documentation of the required 16 hours of annual training. - The record for staff 2 shows documentation of 14 hours of training for the 2017 calendar year. - The record for staff 3 shows documentation of 3 hours of training for the 2017 calendar year. - The record for staff 4 shows documentation of 2 hours of training for the 2017 calendar year. - The record for staff 5 shows documentation of 12 hours of training for the 2017 calendar year. - The record for staff 6 shows documentation of 13 hours of training for the 2017 calendar year. 2 - Staff 2, 3, 4 and 5 were present providing direct care to children during the inspection.

Plan of Correction: Center management will remind all staff that 16 hours of training are required annually. Staff will make up the missing hours during the 2018 calendar year as well obtaining the required 16 hours for the 2018 year.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview the center failed to have a central registry finding within 30 days of employment. Evidence: 1 - The record for staff 7 does not contain documentation of a central registry finding. a. Staff 7 has a documented date of hire as 3/5/18. 2 - Staff 1 confirmed that there is not central registry finding on file for staff 7. 3 - The record for staff 1 contains no documentation of any central registry finding on file. a. Staff 1 has a documented date of hire as 1/5/15. b. Staff 1 stated that she was not sure if the background check had been shredded or if she took it home.

Plan of Correction: Center management will provide documentation that a central registry finding was requested within 10 days of this inspection. Then once the finding is received the information will be forwarded to the inspector with 10 days of receiving the finding.
