Technical assistance was provided in the following areas of the vendor agreement: VENDSUB-000-(9)-031.3.a-k (Lockdown procedures)
A Subsidy Health and Safety inspection was conducted on 4/4/18 from 9:28am to 11:20am to review supplemental health and safety requirements. At the time of entrance there were 11 children ages one year old to eight years old in care with four staff. Children were observed participating in various activities in the classrooms. Records were reviewed for five children four staff . A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-005-B Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The Licensing Inspector reviewed the attendance record for the facility for today, and it did not have the arrival documented for all 11 children present in the facility during the inspection. 2. Staff #4 confirmed that there was not a written record of daily attendance that documented the arrival and departure of each child in care as it occurs. Plan of Correction: The vendor responded: this center is equipped with procure software to clock children in and out of the center, however the director has added as of 4/9/18 an additional paper attendance form in the school age classroom to assure that each child is checked in and out and The director will do a daily attendance check in each classroom via the procure software to assure that the parent indeed did clock the child in correctly.
Standard #: VENDSUB-000-032-A Description: Based on a review of the emergency drill log, it was determined that the vendor did not ensure that the lockdown procedures are practiced annually. Evidence: 1. There was no documentation to demonstrate that the lockdown procedures were practiced during 2017. 2. Staff #4 reviewed the emergency drill log, but was unable to documentation during the inspection to demonstrate that a lockdown drill was completed in 2017. Plan of Correction: The vendor responded: A lockdown drill was done the week after the Florida shooting we had a discussion and showed our children where we would go in case of an emergency it happened the week the children returned back to school the week of March 4th 2018, I apologize that it was not documented but one was done. In the future all lockdowns and drills will be documented at the time of completion.
