Thank you for your time and assistance with the unannounced monitoring inspection conducted today, April 12, 2018. The on-site inspection was from 10:50 a.m. to 3:25 p.m. On this date, at the time of arrival, there were 44 children in three classrooms and in the care of seven staff members. There had been five children in the Morning Mover's program. There were a total of 114 children enrolled in the preschool and before and after-school programs. The exceptions to compliance were noted on the violation notice and the risk ratings were noted on the supplemental page. Five staff members' records were reviewed and five children's records were reviewed. The medication and medication paperwork were reviewed for one children. There were five staff members and four children interviewed. The current theme was farming. Some items observed and reviewed included: injury reports, updated injury prevention plan and emergency preparedness plan, required posted information, first aid kit, emergency supplies, documentation of emergency and shelter-in-place drills, transition time, outdoor play, language art activities, art activities, hand washing and bathroom procedures, lunch brought from home, and nap time. Standard #: 22VAC40-185-130-A Description: Based on review of five children's records and discussion with staff, the center failed to obtain documentation of one child's immunizations before the child's first day of attendance. Evidence: 1. The record of Child #4 obtained documentation of the child's immunizations on 01/30/2018. The first date of attendance for Child #4 was 09/05/2017. Plan of Correction: Staff will ensure that an up-to-date immunization record is obtained before a child's first day of attendance. .
Standard #: 22VAC40-185-140-A Description: Based on review of five children's records and discussion with staff, the center failed to obtain documentation of a physical examination within one month of the first day of attendance for one child. Evidence: 1. The record of Child #4 contained documentation of a physical examination received on 01/30/2018. The first date of attendance for Child #4 was 09/05/2017. Plan of Correction: Staff will ensure that physical examinations are obtained within one month of attendance.
Standard #: 22VAC40-185-140-B Description: Based on review of five children's records and verification from staff, the center failed to obtain documentation that one child had received a physical examination within twelve months of enrollment. Evidence: 1. The record of Child #2 contained documentation of a physical examination completed on 10/30/2013 which was more than twelve months prior to enrollment. The child's date of birth was 09/02/2013 and the date of enrollment was 07/05/2016. Plan of Correction: Staff is now aware that physical exams must be dated within 12 months prior to attendance.
