Comments:
An unannounced monitoring inspection was conducted today, April 16, 2018 to determine the facility's compliance with licensing standards. The inspection was initiated at 10:30 am and concluded at 1:00 pm. Observations were made in three classrooms. Children in the Pre-K3 class rotated from centers (sand table, magnetic blocks, tablet and doll house) to identifying/matching numbers and practice writing skills one on one with staff. Children in Pre-K4 were observed making alligators out of craft sticks and pipe cleaner as the letter "A" was being reviewed. The Pre-K5 class was reviewing the letter "Z", children were drawing items that began with the letter. Children at another table read the book " Salty Seagull" and colored a picture of a seagull. Later children prepared for lunch with hand washing. Children provide their own meals. Staff interacted with children in a positive manner. The three classrooms, bathrooms and hallway were inspected. The playground was not inspected today due to inclement weather. Classrooms are equipped with age appropriate materials and supplies. The first aid kit and non-medical emergency supplies were reviewed and found complete. The facility had the following information posted: the license. The inspector interacted with staff and children in care.

Seven children's records were reviewed along with three staff records.
Medication is on site and was reviewed along with authorization forms.
Staff on site have current CPR, First aid training, daily health observation training and MAT.
The most recent evacuation drill was documented on 03/30/2018. The two shelter-in-place drills for the 2017-2018 school year were completed on 10/05/2017 and 02/14/2018.
Today's Ratios Observed:
Pre-K 3: 8 children with 2 staff
Pre-K 4: 5 children with 2 staff
Pre-K 5: 10 children with 2 staff
School Age children were not present during the inspection.

Standard #: 22VAC40-185-130-A
Description: Based on review of children's records, the facility did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: 1. The record of child #5, enrolled 09/05/2017, did not have documentation of immunizations. 2. The record of child #7, enrolled 09/05/2017, did not have documentation of immunizations.

Plan of Correction: Per the Director: We will request that the parents provide the missing documentation of immunizations for the record.

Standard #: 22VAC40-185-140-A
Description: Based on review of children's records, the facility did not obtain documentation of a physical examination for each child before the child's attendance or within one month after attendance. Evidence: 1. The record of child #5, enrolled 09/05/2017, did not have documentation of a physical examination. 2. The record of child #7, enrolled 09/05/2017, did not have documentation of a physical examination.

Plan of Correction: Per the Director: We will request that the parents provide the missing documentation of a physical examination for the record.

Standard #: 22VAC40-185-510-J
Description: Based on observation, the facility did not ensure all medications were kept in a locked place using a safe locking method that prevents access by children. evidence: Emergency medication for child #7 was observed in a unlocked lunch-box bag. The lunch box bag was zipped and in a backpack that staff carried. The back back was sitting in the staff's chair in the classroom at the time of the observation.

Plan of Correction: Per the Director: We have ordered a safety sack to store the medication in. The medication will be kept in the safety sack to ensure it is inaccessible to children.
