Comments:
An unannounced monitoring inspection was conducted today, from approximately 3:20pm to 5:00pm. 27 children and 3 direct care staff were present. Four staff and 3 children's files were reviewed. See violation notice for missing information. Emergency evacuation drills, shelter-in-place drills, first aid supplies, emergency supplies, and the menu were reviewed, and were all in compliance. The children were observed eating snack, washing hands, transitioning to the gym, and enjoying self-directed activities. Paperwork was left today with site staff. You will have 5 days to complete the corrective actions and return the document to our office.

Description: Based on record review, all children's files did not contain the required documentation. Evidence: 1. 3 out of 3 children's records reviewed did not list the child's first day of attendance (Child #1 and Child #3).

Standard #: 22VAC40-185-240-D-4
Description: Based on record review and medications present at the facility, there is no documentation on site to show that children with emergency medications are in the care of someone who has successfully passed the Medication Administration Training (MAT) training class. Evidence: During today's inspection, no staff present have documentation to show that they have passed the MAT training course.

Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children. Evidence: The cabinet below the sink containing cleaning materials was locked using a bicycle lock that does not prevent access by children.

Description: Based on record review, a medication authorization did not contain all of the required documentation. Evidence: Child #1's medication authorization did not include a parent signature, as required.
