22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted on April 18, 2018, between the hours of approximately 10:20 a.m. and 3:30 p.m. Thirty-eight (38) children were enrolled and thirty-three (33) children were in the care of eight staff members, in five classrooms, during the on site inspection. One additional staff member was present and assisted, as needed. Five children's records were reviewed, one child's immunization record was updated and five staff records were reviewed and/or updated. Two children's medications, as well as authorization forms and administration log, were reviewed. Two children's topical ointments were reviewed. Posted information (license, licensing inspection documentation, emergency evacuation diagram and plan, emergency telephone numbers, menu and classroom schedules), safety equipment (first aid kit, flashlight and radio) and available documents (emergency evacuation drill and shelter-in-place drill records, daily infant forms, attendance records, injury reports, one local agency inspection report and insurance verification) were reviewed/discussed. The daily schedule accurately reflected the activities provided. Lunch and snack were provided by the children's parents and breakfast was provided by the facility. Program observation was completed. Activities observed on this date included: indoor play with classroom materials/toys; stories; diapering, restroom and handwashing procedures; lunch; infant care (feeding and play); naptime and arrival/pick-up departure of children. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Exceptions to program compliance were noted on the violation notice; no repeat violations were cited. Areas of the licensing standards discussed included: documentation - labeling/dating food brought from home and injury report forms; reporting - semiannual written information provided to parents and injury prevention plan; system for obtaining children's immunization updates and emergency plan revisions. Standard #: 22VAC40-185-320-B Description: Based on observation, water temperature measurement taken at the bathroom sink and discussion with staff on April 18, 2018, the program failed to maintain a water temperature that did not exceed 120 degrees F. Evidence: 1. Observation and measurement of the water temperature at the bathroom sink, on this date, established that the hot water measured 133 degrees F. 2. Staff #2 confirmed the water temperature. Plan of Correction: The water temperature will be reduced and will be periodically checked to ensure it is maintained at 120 degrees F or below. Staff members will supervise handwashing until the water temperature is lowered.
Standard #: 22VAC40-185-510-C Description: Based on observation and discussion with staff on April 18, 2018, the program failed to ensure methods to prevent the use of outdated medication were followed. Evidence: 1. Observation of medications, on this date, determined that child G's medication was outdated (expiration date 11/2017). 2. Staff #1 confirmed the expiration date on child G's medication. Plan of Correction: The medication has been sent home.
Standard #: 22VAC40-185-510-G Description: Based on review of two children's medications and discussion with staff #1 on April 18, 2018, the program failed to obtain the prescription label for one child's medication. Evidence: 1. Review of child G's prescription medication, on this date, determined that the medication prescription label had not been obtained. 2. The medication, administered to child G in November, 2017, did not identify the child's name (on the prescription label) in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. 3. Staff #1 confirmed the lack of a prescription label for child G's medication. Plan of Correction: The medication has been sent home. When a current prescription medication is brought to the center, the prescription label will also be obained.
