Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on April 17, 2018 from 11:30 p.m. through 1:30 p.m. There were eighteen children present along with two staff in one classroom. Five children's records were reviewed and updated. Staff records were reviewed at the MACAA office on September 13, 2017. The medication and medication paperwork was reviewed for one child. Areas observed were: lunch, transition, story time, hand washing/bathroom procedures and nap. Interaction between the staff and children were observed as positive and the children appear to be comfortable with the daily routine. Standard #: 22VAC40-185-450-A
Description: Based on observation, the center failed to ensure all children had bottom coverings during nap time. Evidence: 1. Nap time was observed. 2. Eighteen children were napping. Three children did not have bottom coverings. 3. The staff confirmed these three children did not have bottom coverings.

Plan of Correction: Classroom will provide bottom covering for students with only a top sheet
