Background check requirements and the new application were reviewed.

Comments:
The licensing inspector conducted an unannounced mandated inspection from 10:30 until 3:30 p.m. There were 33 children in care with staff supervising at times. Children and infants were observed playing, napping, eating lunch, working a puzzle, and with the computers. Five children's records and six staff records were reviewed. All violations had been corrected. Areas of non-compliance are identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain all the required information in the children's records. Evidence: 1. The inspector reviewed five children's records. Two of five records did not contain all the required information. Child record #4 did not contain info. about chronic conditions, and record # 5 had a social security card for proof of identity.

Plan of Correction: The administrative assistant will get all the required information for the records.

Standard #: 22VAC40-185-510-E
Description: Based on observation, there was a medication that was not labeled. Evidence: 1. The inspector observed a tube of orosol and it did not have a child's name on it.

Plan of Correction: Staff will be told all medications need to be labeled with the child's name.

Standard #: 22VAC40-185-510-N
Description: Based on observation and record review, the center had a medication that had not been picked up by the parent or disposed of within the required time frame. Evidence: 1. The inspector observed a bottle of children's pain and fever medication and the authorization ended 02/18/2018.

Plan of Correction: The medication will be sent home with the parents and parents and staff will be notified/reviewed of the policy.

Standard #: 22VAC40-185-570-E
Description: Based on observation, there was bottle that had not been dated. Evidence: 1. The inspector observed a bottle of formula that had a name on it but it had not been dated.

Plan of Correction: Staff will be told to date and put a name on the bottle.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and interview, the center did not post the latest violation notice. Evidence: 1. The inspector looked for the latest violation notice and it was not posted. Staff looked for the latest violation notice and could not find it.

Plan of Correction: The latest violation notice will be posted in the future.
