Today's visit was a renewal inspection. The inspection began at 9:20am and concluded at1:45pm. There were a total of one hundred twenty children present with eighteen direct care staff and additional support staff were also present to include the Program Director. During the inspection multiple floors of the building were viewed, observations took place in four out of nine classrooms operating today, morning snack time was observed, two medications along with authorizations were reviewed, four injury reports were reviewed, emergency supplies on multiple levels of the building were viewed, nap time was observed, and some polices and procedures were reviewed. The outdoor play areas were not viewed due to rain. Discussion took place regarding documenting what is presented during Daily Health Observation training when conducted "in house" and documenting Program Lead qualifications. Standard #: 22VAC40-185-210-A Description: Based on the inspector's review of ten out of twenty-nine staff records, the center failed to substantiate Program Lead qualifications for a portion of the staff identified as Program Leaders during the inspection. Evidence: Staff members #3, #4, and #5 were identified as Program Leads during the day of the inspection and the records for these staff members do not substantiate required program lead qulifications to include required education, training hours, and programmatic experience. Plan of Correction: The PD will ensure all staff who are Program Leaders have documentation to substantiate their qualifications for their role and title
