A complaint was received by The Central Regional Licensing office on April 2, 2018 regarding supervision of children, procedures for emergencies along with nutrition and food service. The licensing Inspectors conducted an unannounced inspection in response to the complaint on April 27, 2018. Staff were interviewed, documentation of injury reports and food service, and staff and children's records were reviewed. Based on the information gathered during the investigation, the preponderance of evidence does not support the allegation. Therefore the complaint is determined to be not valid. Violations were cited that did not pertain to the complaint (see violation notice).
The inspectors were on site from 10 am until 11 am.

Standard #: 22VAC40-185-60-A Complaint related: No Description: Based on review of children's records, the facility did not obtain all required information for each child's record. Evidence: 1. The record of child #1 did not have the addresses for the two emergency contacts listed. 2. The record of child #2 did not have the addresses for the two emergency contacts listed. The name address and phone number for two emergency contacts is required. Plan of Correction: Per the Director: All addresses will be present prior to enrollment.
Standard #: 22VAC40-185-210-A Complaint related: No Description: Based on review of staff records, the facility did not ensure staff assigned as program leaders met program leader qualifications. Evidence: Staff #1, identified as a program leader since July of 2017, did not meet the qualifications of a program leader. Plan of Correction: Per the Director: Staff will be required to meet all training prior to advancing to lead position.
Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation, the facility did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1, The mini blinds in the infant classroom were broken. The blinds were extended down to the window sill approximately a foot from the floor. 2. The broken blinds created a loop that could entangle a child. Plan of Correction: Per the Director: We will replace the mini blinds by Monday.
