An unannounced renewal inspection was conducted today from 9:15am-1:15pm. There were 54 children (3-5yrs.) supervised by 8 direct care staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed eating morning snack, discussing the weather during circle time, preparing for a fieldtrip, and playing on the playground. Hand-washing procedures were also observed. Children's lunches are brought from home. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, 1 of 6 children's records did not contain all of the required information. Evidence: Child #6's file did not contain documentation of a second emergency contact. Plan of Correction: Will request the second emergency contact from the parents.
Standard #: 22VAC40-185-70-A Description: Based on record review, 1 of 5 staff records did not contain all of the required information. Evidence: Staff A's record did not contain documentation of contact with two reference checks. Plan of Correction: Will complete two background checks for the employee.
Standard #: 22VAC40-185-210-A Description: Based on record review, 1 of 2 staff did not have documentation of lead teacher qualifications. Evidence: Staff D's record did not contain documentation for qualifications to be a lead teacher. Plan of Correction: I will request the documentation from the staff.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: In the large Early Childhood Center classroom, on the 3's side, Lysol wipes and Clorox bleach spray was found in an unlocked cabinet above the sink area. Plan of Correction: The cleaning supplies will be relocated to the locked closet.
Standard #: 22VAC40-185-510-N Description: Based on observation, it was determined that all expired medications were not picked up by the parent within 14 days of expiration, and were not disposed of by the center.
Evidence: Child #5's Epi-pen was on site at the time of inspection (5/1/2018), and expired 12/2017.
Plan of Correction: Will be returned to the parent.
Standard #: 22VAC40-185-560-G Description: Based on observation, all food containers brought from home were not properly labeled to identify the owner and the date. Evidence: Child 1's lunch bag was not labeled with the child's name and current date. Plan of Correction: Will ensure that all children's lunches are labeled with the child's name and current date.
Standard #: 22VAC40-191-40-D-3-D Description: Based on record review, 1 of 5 staff did not have current background checks within the last 3 years. Evidence: Staff E's central registry check expired on 10/6/2013. Staff E's criminal history check expired on 9/6/2013. Plan of Correction: Staff will obtain current background checks.
