Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Statement (SDS) for new staff hired after 7/1/2017 and requesting out-of-state Central Registry checks for staff who have lived outside of Virginia within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out-of-state central registry forms. VA DSS will be paying for initial child care finger print checks until 9/30/2018. Each facility must establish a point of contact with VA DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligibility letters will be sent out by e-mail and staff may not be hired until the completed letter is received. Please read, review, and respond to all correspondences regarding the fingerprint roll out. Discussed with center management medications, background checks and staff records. Also discussed the new renewal application.

Comments:
An unannounced monitoring inspection was conducted from 9:25 am-11:45am. 105 children were in care with 13 staff members present. Children were observed engaged in group activities such as learning about the letter"P", doing gardening activities, outdoor play and leaving on a field trip. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A
Description: Based on record review, center did not maintain complete records for each staff person. Evidence: Staff # 1's file only had one reference.

Plan of Correction: We will obtain the second reference today.

Standard #: 22VAC40-185-240-A
Description: Based on record review, center did not maintain orientation documentation for staff. Evidence: 1. Staff # 1 was hired on 6/21/17. There was no orientation documentation found in the record. 2. Staff # 2 was hired on 1/16/18. There was no orientation documentation found in the record. 3. Staff # 3 was hired on 6/13/16. There was no orientation documentation found in the record. 4. Staff # 4 was hired on 3/20/18. There was no orientation documentation found in the record.

Plan of Correction: We will complete the orientation documentation and place in each staff's file.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, center did not obtain a central registry search for employees within 30 days of employment. Evidence: 1. Staff # 4 was hired on 3/20/18. 2. Staff # 5 stated that she thought the fingerprinting would eliminate the need for a Virginia Central Registry search. 3. Center did not obtain a central registry search for staff # 4 within 30 days of employment.

Plan of Correction: We will submit the form today.

Standard #: 63.2-1720.1-B-3
Description: Based on record review, center did not obtain an out of state central registry search for staff that has lived outside of Virginia within the last five years. Evidence: Staff #1's record indicated that he has lived outside of Virginia within the last five years. The center did not obtain an out of state central registry search for Staff # 1.

Plan of Correction: We will submit the form as soon as possible.
