Discussed medication authorizations and allergy care plan forms.

Today an unannounced Monitoring Inspection took place between the hours of 10:30am and 2:45pm. At the start of the inspection there were a total of 103 children with 19 staff; all ratios were incompliance. A total of 5 children?s files and 7 staff files were reviewed. The children were observed playing outside, washing their hands, being read to, eating lunch (turkey pizza, mixed salad, green beans and tropical fruit) and starting their naps. This week the school is hosting a book fair. An exit interview was conducted with the program Director and the District Manager. Standard #: 22VAC40-185-240-D-1
Description: Based on observation and interview, the center has agreed to administer a prescribed medication to one child without a staff member on-site that has satisfactorily completed a training program that qualifies them to administer the medication. Evidence: Child #9 has an emergency medication on-site to be administered in case of an accidental ingestion of an allergen. There are no staff members on-site that have been trained to administer this particular medication.

Plan of Correction: MAT certified staff members were present on site to administer medication and Auvi-Q training has been obtained by additional managers.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1. There is reptile-calcium located in a cabinet under the reptile cage in the K-Stem. The cabinet does not contain a lock. 2. There is an over-the-counter contact lense solution located on an open shelf in the K-Stem classroom.

Plan of Correction: All substances were moved to a locked cabinet during the inspection.

Standard #: 22VAC40-185-520-A
Description: Based on observation, other-the-counter skin products were kept beyond their expiration date. Evidence: Child #1 sunscreen expired 3/2018. Child #7 sunscreen expired 1/2018. Child #8 sunscreen expired 3/2018.

Plan of Correction: All over the counter skin products were given back to the parents or disregarded on the day of the inspection.

Standard #: 22VAC40-185-560-F
Description: Based on observation, a current weekly food menu was not posted. Evidence: The menu that was posted was dated for the previous week.

Plan of Correction: Current week menu was posted during the inspection.

Standard #: 22VAC40-185-560-G
Description: Based on observation, food brought from home was not dated and labeled in a way that identifies the owner . Evidence: Child #1 had a baggy of cereal in their cubby of the Preschool 1 classroom. The baggy was not labeled with the child's name or today's date. Child #6 had a tote bag filled with snacks in the refrigerator of the Older Infant classroom. The bag and the snacks were not labeled with the child's name or today's date.

Plan of Correction: Retrained teachers and informed parents of proper labeling of outside products coming into the center.
