63.2 Facilities and Programs..

Comments:
The inspector conducted an unannounced mandated inspection from 10:30 a.m. until 1:15. There were 18 children in care with three staff supervising at all times. Children were observed playing with play dough, playing with and cleaning up toys, going to the gym and playing in the gym and lining up for lunch. No medication was reported being administered at this time. The violation from the last inspection has been corrected. Staff reported that the playground is no longer being used but outside time is provided in other ways. Violations are identified on the violation notice. Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not have immunizations on file. Evidence: 1. The inspector reviewed five records. Three of five records (#1#3, #4) did not have immunization records on file.

Plan of Correction: Some records may be at the main office. Main office will send the records to the classroom.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not have physical examinations for all the children. File #1, #3 and #4 did not have physical exams and they had been enrolled longer than 30 days.

Plan of Correction: Staff will check with main office to see if the examinations are there.

Standard #: 22VAC40-185-60-A
Description: Based on record review the center did not have all the required information in the children's records. Evidence: 1. The inspector reviewed 5 records. Record #1 did not have chronic conditions, allergies, or emergency addresses listed on the form. Child #2 had an allergy listed but not action to take listed.

Plan of Correction: Staff will get he information from the parents.
