Comments:
A Subsidy Health and Safety inspection was conducted on 5/1/2018 to review supplemental health and safety requirements. The inspector arrived at 2:00 PM and departed at 4:00 PM. Twenty-two children were in care with four staff. It was reported that no currently enrolled children have diagnosed food allergies. A sample size of five staff and six children's records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based upon review of the facility's emergency preparedness plan, the facility has not ensured that the written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures. Evidence: 1. The lock down procedures in the provided emergency preparedness plan ended with an incomplete sentence and remaining procedures for re-unifying children with parents following an actual lock down emergency. 2. The written procedures for evacuation of children and sheltering-in-place did not include written procedures for re-unifying children with parents following an actual evacuation and shelter emergency.

Plan of Correction: The facility responded with the following: Reunification procedures will be added to evacuation, shelter-in-place and lock down procedures.
