Thank you for your time and assistance with the unannounced monitoring inspection conducted on May 3, 2018. The on-site inspection was from 9:55 a.m. to 12:45 p.m. On this date, there were 19 children present and in the care of two (2) staff members. There were a total of 20 children enrolled in the preschool program. The exceptions to compliance were noted on the Violation Notice and the risk ratings for those violations were noted on the Supplemental Page. There were five (5) children's records reviewed on this date, and two (2) staff members' records were reviewed on September 13, 2017 at MACAA's main office. Two (2) staff members and two (2) children were interviewed. The medication and medication paperwork were reviewed for two children. Lunch was provided by the school and consisted of spaghetti with meat sauce, bread sticks, lettuce salad, strawberries, and milk. The children were working on an art project, playing and exploring during center time. Some items reviewed/observed were outside play time, circle time, transitions, hand washing, lunch time, nap time, injury reports, first-aid kit and emergency supplies, documentation of evacuation and shelter-in-place drills, and required posted information. Standard #: 22VAC40-185-90--A Description: Based on a review of five children's records and verification from staff, the center failed to obtain one of the required written parent agreements. Evidence: 1. The record for Child #4 did not contain a statement that the parent will inform the center within 24 hours after the child or any member of the immediate household has developed a reportable communicable disease. Plan of Correction: Staff Member A will contact the Family Advocate and ensure that the written parent agreement is obtained.
Standard #: 22VAC40-185-450-A Description: Based on observation and confirmation from staff, the center failed to provide a bottom linen cover during rest time. Evidence: 1. Three children's cots did not contain a bottom linen cover. Plan of Correction: Staff corrected during the inspection and in the future will ensure that each child has a bottom linen cover and a top linen cover to be used during nap time.
Standard #: 22VAC40-185-550-M Description: Based on review of one injury report and discussion with staff, the injury report did not contain all required information. Evidence: 1. The injury report for Child #5, dated 01/10/2018, did not contain the time the injury occurred, the time the parent was notified of the injury, and documentation of further action to be taken to prevent recurrence of injury. 2. Staff Member A confirmed that the injury report was not complete. Plan of Correction: Staff is aware of what is required on an injury report and will make sure that the report is complete in the future.
