Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:00am - 12:45pm. There were twenty one children in care with five staff: four infants with one staff, three toddlers with one staff, seven two year olds with two staff and seven children ages three and four with one staff. One additional staff arrived during the inspection. The children were observed at free play, coloring, listening to music, playing dress-up and during lunch. Five staff's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. Two of five records reviewed (children's records #2 and #4) did not address any chronic physical problems and did not have street addresses for their emergency contacts.

Plan of Correction: Staff said they would request this information from the children's parents at pick up time. The children's records will be reviewed to ensure all information is complete.
