22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal inspection was conducted on 05/10/2018 from 11:20am - 3:25pm. At the time of the tour there were 33 children in care with eight staff. The children ranged in age from infant - four years. Children were observed during morning program time, lunch and nap. Records were reviewed for eight children and seven staff. Three medication reviews conducted. The violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to ensure that equipment of the center shall be maintained in a safe condition. Evidence: 1. Two cribs had a masking tape name label placed on the inside area of Plexiglas. Two cribs had paper name cards affixed to the inside area of Plexiglas using masking tape. a. Three of these cribs were occupied by sleeping infants. b. This is a choking hazard. All of these name cards & tape are accessible to the children in the cribs. 2. An infant was observed napping in a crib with a loose fitting crib sheet. a. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death. Plan of Correction: The name cards and tape will be removed today. A tight fitting crib sheet will be placed on the crib.
Standard #: 22VAC40-185-280-B Description: Based on review, the center failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A small 2 oz. bottle, that was less than half full, was observed in a high cabinet in the infant/toddler class. a. The bottle was labeled with multiple warnings indicating the contents as hazardous. Plan of Correction: This was discarded during the inspection. Staff will be reminded to keep all products of this nature in a locked cabinet.
Standard #: 22VAC40-185-540-C Description: Based on observation, the center failed to ensure that the required first aid kit shall included all of the required elements. Evidence: 1. The bus used to transport children from a local public school did not have a complete first aid kit. The bus kit lacked the following: tweezers, band-aids, antiseptic cleansing solution/pads, thermometer, and single use gloves 2. Administrative staff verified these items were not on the bus. Plan of Correction: These items will be obtained and placed in the bus today.
Standard #: 22VAC40-185-580-C Description: Based on observation the center failed to ensure that during transportation of children that emergency numbers shall be in the vehicle. Evidence: 1. The numbers for poison control and 911 were not in the bus used to transport children from a local public school - observed during the inspection. 2. Administrative staff verified these numbers were not in the vehicle. Plan of Correction: These numbers will be added to the bus today.
