Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted May 7, 2018. The on-site inspection was from 2:05p.m. to 5:05p.m. On this date, there were 24 children in three classrooms and in the care of five staff members. There were two additional staff members available to assist if needed. The total enrollment for the program was 44 children. There were five staff members' records and five children's records reviewed. There was no medication on-site or being administered, which is the policy of the provider. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were noted on the supplemental page. All interactions between the staff and the children were age-appropriate and positive. Snack consisted of goldfish crackers and apple juice. Some items observed and reviewed during the inspection included nap time, transitions, hand washing and bathroom procedures, snack, diaper changing procedures, dismissal procedures, injury reports, outdoor play area, diaper ointments, documentation of evacuation drills, and required posted information which included the license, inspection summary, snack menu, daily schedule, emergency evacuation maps/plan, and emergency phone numbers. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of five staff records and verification from the director, the center failed to obtain two references as to character, reputation and competency for one staff member. Evidence: 1. The record for Staff Member #4 did not contain documentation that two references were checked. Date of hire was 10/14/2016.

Plan of Correction: The director will contact two references for Staff Member #4 by tomorrow.

Standard #: 22VAC40-185-340-D
Description: Based on observation, record review, and verification from the director, the center failed to have qualified program leaders supervising two different groupings of children. Evidence: 1. Staff Member #2 was the only staff member present and working in Classroom C with a group of seven children from 2:05p.m. to 4:30pm. on 05/07/2018. There was no documentation that Staff Member #2 met the qualifications to be a program lead. 2. Staff Member #1 was the only staff member present and working in Classroom D with a group of four children from 2:05p.m. to 4:30pm. on 05/07/2018. There was no documentation that Staff Member #1 met the qualifications to be a program lead.

Plan of Correction: The director has developed a plan so that a qualified program lead is always present with each grouping of children. In the meantime, Staff Members #1 and #2 will be working toward the requirements to become qualified as program leads.
