An unannounced monitoring inspection was conducted beginning at 6:50 a.m. and ending at 7:45 a.m. with the director and one staff. There were eleven children in care ranging in age from five to ten years of age. Children were observed at their choice of centers that included board games, hula hoops and legos. Three staff records and five children's records were reviewed. Emergency drills were documented and up to date. Menus were posted and up to date, including any substitutions. Standard #: 22VAC40-185-140-A Description: Based on review of documentation, the center failed to obtain documentation of a physical examination within thirty days of attendance. Evidence: Child #3, enrolled 9/19/2017, did not have documentation of a physical in the file. Plan of Correction: The school nurse will be contacted for this documentation this morning.
