22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted with the area manager beginning at 3:45 PM and ending at 5:45 PM. There were twenty-five school-aged children in care during the inspection. The children were observed playing card and board games, drawing, and playing with manipulatives. The children were also observed during outdoor recreations time and doing homework. Ten children's records and two staff records were reviewed and in compliance. Two medications were reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The emergency escape log was reviewed. The most recent emergency escape drill was documented as 2/14/18. The most recent shelter-in-place drill was documented as 11/31/17. The most recent lockdown drill was documented as 1/25/18. The most recent health inspection was documented as 5/1/17. Standard #: 22VAC40-185-260-A
Description: Based on a review of documentation, the center did not have documentation of an annual fire inspection report from the appropriate fire official. Evidence: Documentation of an annual fire inspections was not available during the inspection.

Plan of Correction: The center obtained the latest Building Inspection from the school Office.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not keep all hazardous substances in a locked place using a safe locking method that prevents access by children. Evidence: A jug of cleaning solution and bleach were in an unlocked closet which was accessible to children. The labels of both cleaning products stated keep out of reach of children.

Plan of Correction: The center will be retraining the current staff on health and safety and keeping the door to the custodial closet locked at all times .

Standard #: 22VAC40-185-340-B
Description: Based on observation and staff interviews, the center did not one adult on the premises in charge of the administration of the center during the all of the center's hours of operation. Evidence: On April 19, 2018 from3:45 to 4:15 there was no staff in charge of the administration of the center on the premises.

Plan of Correction: The center has placed an interim director in charge of administration of the site for the remainder of the school year . The interim director's qualifications have been provided to licensing.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation and staff interview, the center did not have the appropriate staff-to-child ratio. Evidence: On April 19, 2018 from 3:45 PM to 4:15 PM there was one staff member supervising 25 children ages 6 through 11 years old.

Plan of Correction: The center will adhere to staff /child ratio at all times. There will be at least 2 staff present to provide care and supervision of the 29 students .

Standard #: 22VAC40-185-510-C
Description: Based on a review of documentation, the center did not have written authorization from the child's physician and parent for long term prescription drug use or over-the-counter medication. Evidence: Child A's (DOB 12/15/06) did not have written authorization from the physician and parent for a long term prescription medication.

Plan of Correction: The center will provide parent with the 10 day authorization form until parent provides us with the long term form.

Description: Based on review of documentation and staff interviews, the center did not implement practice evacuation drill on a monthly basis as required. Evidence: The most recent emergency evacuation drill was documented as February 14, 2018.

Plan of Correction: Retrain the staff on drill policies, April Drills have been conducted, staff will complete all missing drills in the month of May.
