22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from approximately 9:40am-11:50am. There were 53 children in care directly supervised by 9 staff. A sample of 7 children?s records, 4 staff records and 1 medication was reviewed. The children were observed doing a project, on the playground, reading, counting, during bathroom procedures and washing hands. Zero new staff have been hired since the last inspection. Standard #: 22VAC40-185-60-A Description: Based on record review, children's records were incomplete. Evidence: Of 7 records reviewed, 5 were missing required information. Emergency contacts for all children reviewed were missing addresses. 1) Child B was missing proof of identification. Date of enrollment was 9/2017. 2) Child C was missing mother's employer and employer phone number, and proof of identification. Date of enrollment was 3/5/2018. 3) Child D was missing proof of identification. Date of enrollment was 9/2017. 4) Child E was missing father's employer phone number, proof of identification and 1 of 2 required emergency contacts. Date of enrollment was 9/2017. 5) Child F was missing father's employer phone number and proof of identification. Date of enrollment was 9/2017. Plan of Correction: Child B is 6+ years old. Children C, D, E and F parents were requested for the information.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1) Disinfectant spray was found on top of a cabinet in the ladies bathroom. 2) Paint, primer, wood cleaner, disinfectant, WD-40 and insulating foam were found in an unlocked closet located in the boys bathroom. Plan of Correction: Lysol removed, lock was placed on closet.
Standard #: 22VAC40-185-290-3 Description: Based on observation, electrical outlets were not protected with protective covers. Evidence: 1) Two electrical outlets were not covered on the wall near the entrance into the building. 2) One electrical outlet was not covered on the bottom of the wall near the steps leading towards the office. 3) Two electrical outlets were not covered on a pillar located in the after care area. 4) One electrical outlet was uncovered on the wall near the radio. 5) One electrical outlet was uncovered on a wall near the classroom. Plan of Correction: Outlets were covered.
Standard #: 22VAC40-185-340-F Description: Based on observation, children under the age of 10 were not in actual sight and sound supervision of staff. Evidence: 1) Two children were working together on a project in the after care area. There were no staff around to supervise the children. 2) Two children were found traveling alone from the playground to the bathroom located on inside the building. There were no staff supervising the children upon entrance into the building and they out of sight and sound supervision of the staff on the playground. Plan of Correction: Licensing inspector discussed sight and sound supervision with staff. A teacher began to supervise the children doing the project and the lead teacher relayed the supervision requirements to the assistants on the playground.
Standard #: 22VAC40-185-510-E Description: Based on observation, required labels were missing from medication. Evidence: Medication for Child A was reviewed, the dosage amount and the times to be given were not labeled on the medication. Plan of Correction: Parents were contacted and label was corrected.
Standard #: 22VAC40-185-510-J Description: Based on observation, medication was not kept in a locked place. Evidence: Medication was found in a cabinet that was ajar. The door was open and unlocked during the inspection. Plan of Correction: Medication was moved to a locked box during inspection.
Standard #: 22VAC40-185-560-G Description: Based on observation, food brought from home was not labeled correctly. Evidence: 8 lunch boxes, not labeled with a name and date, were found outside on a table near the trailer classrooms. Plan of Correction: Labels have been re-attached to lunch boxes.
