Comments:
An unannounced monitoring inspection was completed on 05/14/2018. There were 34 children, ages 4 years-12 years, and 4 staff providing direct supervision present during today's inspection. The inspector reviewed 5 children's records, 5 staff records and 5 medications during the inspection. The children were observed in the following activities: arrival to the program, homework time, outdoor play, afternoon snack and preparing to participate in the daily scheduled activity of Kung-Fu. The inspector observed proper hand washing and table sanitization during the inspection. The inspector reviewed the emergency supplies during today's inspection. The inspector discussed the following with site staff: the new forms available to providers, summer day camp, upcoming expiration dates on medications, and annual training hours. An exit conference was held at the conclusion of the inspection. The inspector arrived at 2:00 p.m. and departed at 5:10 p.m. Thank you for your time today. Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with staff, the facility failed to ensure that all children's records contain the required components as stated in the standards. Evidence: 1. The record for Child #1 did not contain the following information: the name and phone number of the child's physician, allergies and intolerance to food, medication, or any other substances, and the actions to take in an emergency situation, health information as required by 22VAC40-185-130 and 22VAC40-185-140, the previous child day care attended y by the child, and the name of any additional programs or schools that the child is concurrently attending. 2. The record for Child #2 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the second contact listed for Child #2 did not have a complete address. 3. The record for Child #3 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the contacts listed for Child #3 did not have a complete address. 4. The record for Child #4 did not contain the name of any additional programs or schools that the child is concurrently attending. 5. The record for Child #5 did not contain the name of any additional programs or schools that the child is concurrently attending.
