Technical Assistance:
Discussed the following with the Director: - fingerprinting legislation requirements - having expired epi-pens on site.

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 6 staff files, 5 children's files, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 3:30-5:30pm. There were 64 children under the supervision of 6 staff. During the inspection, children were observed playing outside, working on craft projects and homework, working with clay, and enjoying free play. Today, the children were served tater tots and dices peaches, with the choice of white milk, chocolate milk or water. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 3 staff certified in MAT (medication administration training). Standard #: 22VAC40-185-140-A
Description: Based on file review, 2 out of 5 children's records do not have physical examinations. Evidence 1. Children #1 and #4 do not have physicals on file.

Plan of Correction: The Director will ask the parents for these.

Standard #: 22VAC40-185-60-A
Description: Based on file review, 5 out of 5 children's records do not have all required information. Evidence 1. Children #1-5 do not have parent place of employment addresses. 2. Child #3 does not have the parent's place of employment.

Plan of Correction: The Director will work on updating the registration form. She will ask child #3's parent for this information.

Standard #: 22VAC40-185-510-C
Description: Based on observations, the center failed to follow all procedures for administering medications. Evidence 1. Child #6 has an authorization for emergency medications, albuterol, Benedryl and an epi-pen. Only the epi-pen was brought into the center to be used in case of emergency. 2. Child #7 has Tylenol on site, which expired o2/18.

Plan of Correction: The director will talk to the parents about getting required medications brought into the center.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on file review, 1 out of 5 staff records does not have a criminal history record check completed within the past three years. Evidence 1. Staff #1's most recent criminal history record check was completed on 12/1/14.

Plan of Correction: Staff #1 will have a fingerprint check completed.

Standard #: 63.2-1720.1-B-3
Description: Based on file review, 1 out of 5 staff does not have a search of the central registry completed for a state in which the individual has resided in the preceding five years. Evidence 1. Staff #4 has information on the completed sworn disclosure statement of having lived in another state within the past 5 years. There was no search of the central registry completed for this state.

Plan of Correction: The Director will complete this.
