An unannounced Renewal Inspection was conducted on May 15, 2018 from approximately 6:55 am until 9:00 am. Upon entrance there were 19 children in care and 2 staff present with children. The record review sample consisted of 5 children's records and the on site required documentation for 5 staff records. Six medications were observed and reviewed. Children were observed playing in centers to include board games, home-living, cars and dolls. Children were also observed eating breakfast and playing large group games. The results of the inspection were verified and reviewed with the center director during the exit interview. Areas of non-compliance are noted on the violation notice. Standard #: 22VAC40-185-60-A Description: Based on a record review of 5 children's records and staff interviews, the licensee did not ensure the center shall maintain and keep at the center a separate record for each child enrolled which shall contained required information. Evidence: During the inspection on May 15, 2018 3 out of 5 children's records wer determeind to be inomplete. 1) The record for child #1 did not include the home address of 1out of 2 of the emergency contatacts listed. 2) The record for child #2 1 did not include the appropriate legal paperwork on file when the custodial parent requests the center not to release the child to the other parent. The center director confirmred the name listed as not authorized to pick up was one of the parents of child #2. 3) The record for child #5 does not include a copy of physical in the child's record within one month of attendance. Child #5 first attended on 9-5-2017. Plan of Correction: The Center Supervisor will ensure all children's records are complete and up- to- date.
Standard #: 22VAC40-185-540-D Description: Based on observation and staff interviews, the licensee did not ensure all required emergency supplies were at the center and shall be available on field trips. Evidence: During the inspection on May 15, 2018, there was not one working, battery-operated radio in each building used by children. Plan of Correction: The Center Supervisor will ensure all battery operated radios are in working condition.
Standard #: 22VAC40-185-560-K Description: Based on observation the licensee did not ensure tables were sanitized before and after each use for feeding. The definition of "Sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution. Evidence: During the inspection on May 15, 2018, children were observed eating their morning snack and the table was not sanitized before or after the children were eating. Plan of Correction: The Center Supervisor will ensure the breakfast table is cleaned and sanitized before and after children eat.
Standard #: 22VAC40-185-560-N Description: Based on observation the licensee did not ensure no child shall be allowed to drink or eat while walking around. Evidence: During the inspection on May 15, 2018, child #6 was observed eating their morning snack (pancakes and bacon) and was walking around the center while eating. Plan of Correction: The Center Supervisor will ensure children are waiting to eat until they are seated in the appropriate area.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the licensee did not ensure that the findings of the most recent inspection of the facility were posted in a conspicuous place for the public to view. Evidence: During the inspection on May 15, 2018 the results of the inspection were not posted for the public to view. The center director confirmed they were stored in a notebook. Plan of Correction: The cetner supervisor will ensure the most recent licensing inspection findings are posted.
