22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today from 9:30am-12:45pm. There were 27 children (3-5yrs.) supervised by 3 direct care staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed building with clay, drawing on the chalkboard, polishing, buttoning, completing puzzles, pouring, counting, and rolling Montessori mats. Hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Description: Based on observation, children's records were not treated confidentially. Evidence: A child allergy list was posted in the Preschool classroom during the time of inspection without a cover sheet, or other means of treating allergy information confidentially.

Description: Based on record review, 1 of 4 children's records did not contain documentation of complete emergency contact information. Evidence: Child #1's (start date: 6/9/2016) record did not contain documentation of addresses for either of the two required emergency contacts.

Description: Based on record review, 1 of 3 staff records did not contain documentation of 2 completed reference checks, as required. Evidence: Staff #C's (hire date: 11/1/2016) record did not contain documentation of 2 completed reference checks before employment.

Plan of Correction: Staff member not a teacher. Forms have been updated.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. An opened bag of potting soil was observed sitting on the floor in the Preschool napping room. The potting soil contained a label that stated "Keep out of reach of children". 2. 3 spray bottles of cleaning solution, and 1 spray bottle of hydrogen peroxide were observed on counters in the Toddler classroom.

Plan of Correction: The potting soil was removed from the napping classroom during the time of inspection. Class was not in session (Friday). Chemicals were secured before next class day. The spray bottles were put away in a locked cabinet during the time of inspection

Description: Based on observation, children under 10 years of age were not within actual sight and sound supervision of staff at all times, with the exception of using the restroom. Evidence: During the time of inspection, children in the Preschool classroom were observed walking out of the classroom, through an open classroom, and into the napping classroom across the hall, without the supervision of staff.

Plan of Correction: Door is to be closed when teacher not inside classroom.

Description: Based on observation, prescription medication was on site during the time of inspection without parental authorization. Evidence: Child #4's Auvi-Q emergency medication was on site during the time of inspection without an accompanying completed medication authorization form.

Plan of Correction: Parent has sent in new Epi-pen

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications, all of the center's procedures for administering medication were not followed. Evidence: Child #4's Epi-Pen was on site during the time of inspection, which expired 12/2017.

Plan of Correction: Expired Epi-pen sent home.

Standard #: 22VAC40-185-550-D
Description: Based on record review and staff interview, the center did not maintain documentation of monthly evacuation drills. Evidence: There was no documentation of a fire drill conducted in January 2018.

Plan of Correction: It has been made up.
