Comments:
A Subsidy Health and Safety inspection was conducted on 4/12/18 to review supplemental health and safety requirements. Licensing Inspector arrived at 10:30 and departed at 11:45 AM. At the time of the inspection, there were nine children, ages infant through preschool in care with two staff. Children were observed participating in free play and outdoor play. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: VENDSUB-000-012-E
Description: Based on review of staff records, it was determined that the vendor did not ensure that all staff complete cardiopulmonary resuscitation (CPR) and first aid within the first 30 days of employment. Evidence: Staff #4, date of hire 9/4/17, did not have documentation of completing CPR and first aid within the first 30 days of employment.

Plan of Correction: Vendor will ensure that CPR and first aid are accomplished for staff #4 and that all new staff complete training within the first 30 days of employment.

Standard #: VENDSUB-000-032-A
Description: Based on review of documents at the facility, it was determined that the vendor did not ensure that lockdown drills were practiced a minimum of once annually. Evidence: There was no documentation that a lockdown drill has been conducted.

Plan of Correction: Vendor will ensure that all drills are conducted as required and documented.
