Consultation was given on MAT Avui-Q training, allergy action plans matching medication on-site and background check changes.
An unannounced monitoring visit was conducted from 10:00-12:45. All rooms were in ratio with direct care staff. Children were observed participating in circle time, transition to the outside playground area, outside art class, Montessori center times and activities. Physical space, medication, emergency preparation, asbestos management, a sample of children's records and nutrition were reviewed. Lunch today is ravioli, peas, fruit and milk. Three staff have current Medication Administration Training (MAT) certification as emergency medications are on-site. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not stored in a locked place. Evidence: Air freshener was accessible to children in the downstairs bathroom leading to the playground as well as in the upstairs hall bathroom. Plan of Correction: The air freshener was moved at the time of the inspection and will be placed in teacher closets.
Standard #: 22VAC40-185-290-3 Description: Based on observation, two electrical outlets did not have protective covers. Evidence: Two outlets in the hall way between the boys and girls bathroom in the "Toddler" space did not have outlet covers. Plan of Correction: Staff were going to replace the missing outlets during the inspection.
Standard #: 22VAC40-185-510-A Description: Based on medication review, one of ten medications was missing parent authorization. Evidence: Medication for child #1 did not have authorization completed by the parent allowing the center to administer an emergency medication. Based on medication review, one of ten medication authorizations was incomplete. Evidence: The written parent authorization was missing the effective dates and the date the parent signed the authorization allowing the center to administer an emergency medication for child #2. Plan of Correction: Authorization forms will be updated.
Standard #: 22VAC40-185-510-C Description: Based on medication review, one of ten medications was missing a long term authorization completed by the physician. Evidence: No authorization was available to verify that the physician had completed the authorization for a long term emergency medication (on-site longer than 10 days) for child #1. Plan of Correction: The center administration reached out to the parent to have the physician complete the authorization.
Standard #: 22VAC40-185-550-D Description: Based on documentation and confirmed by center administration, a monthly evacuation drill was not completed. Evidence: A shelter-in place drill was conducted for 12/2017 in place of an evacuation drill rather than in addition to the monthly evacuation drill. Plan of Correction: Moving forward, center management will ensure an evacuation drill is conducted each month.
