On 5/16/2018 an incident regarding behavioral management was self-reported. An unannounced focused investigation was conducted today between the hours of 8:40am and 2:00pm. Based on the preponderance of evidence gathered, violations were cited. The findings from today's visit can be found on the Violation Notice. Description: Based on interview, the licensee did not use behavioral guidance that was constructive in nature and redirect children to appropriate behavior. Evidence: On 5/16/2018 at approximately 5:45pm, the three year old, four year old and school age classrooms were combining with Staff #1 and Staff #2. Staff #1 began to name-to-face checks by calling the children's names and the children respond with "here" or "present". When Staff #1 called Child #1's name, the child responded with "chicken nugget". Staff #1 then walked up to Child #1 and pinched their ear and said "Why did you say that? Do you hear what I'm saying?" Interviews with Child #1 and Staff #2 confirmed that Staff #1 did pinch Child #1's ear after they said "Chicken nugget" during the name-to-face checks.

Plan of Correction: The employee will be terminated. Staff will be retrained on child abuse policies.
