Technical Assistance:
Inspector spoke with Director about the updated background check regulations during this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 11:10am-12:55pm. At the time of inspectors' arrival there were 65 infant to kindergarten aged children in care with 14 staff members and 2 administrators. Children were observed participating in lunch, indoor play, transitioning between activities, diapering, handwashing, nap, and educational activities. A sample of 7 children's records and 7 staff records were reviewed. Medications and injury reports were also reviewed. The areas of noncompliance are detailed in the violation notice and were discussed with the director during the exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: The record for child #5 did not contain updated immunization documentation. The documentation available was dated 2/10/17 (four months before the child's 2nd birthday).

Plan of Correction: The center requested update immunization documentation from the parents.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: 1. Outside on the playground there were multiple areas within the fall zones of the poured in place resilient surfacing for the play equipment that were cracked or in disrepair; presenting potential injury hazards. 2. In room #6(4 year olds) there was a 12" piece of rusty metal in the bottom of the unlocked cabinet below the sink; presenting potential injury hazards. 3. In room #5(3 year olds) there was an open box of vinyl gloves in the restroom at child level; presenting potential choking/suffocation hazards.

Plan of Correction: 1. Repairs to the poured in place surfacing will be scheduled. 2. Rusty metal was removed during the inspection. 3. Vinyl gloves were moved during the inspection.

Standard #: 22VAC40-185-550-M
Description: Based on record review it was determined the center did not ensure that each written record of children's serious and minor injuries contained all of the required components. Evidence: The injury report dated 4/23/18 for child #5 did not contain the treatment given, and the date, time and method of parental notification.

Plan of Correction: Staff will be retrained on completing injury reports.
