Comments:
An unannounced monitoring inspection was conducted from 9:50am-1:15pm. At the time of entrance, 93 children were in care with 18 staff members present. Children were observed during snack time, circle time, preparing for graduation, engaged in group activities, counting in Spanish and English, and outdoor play time. A selection of staff and children records, emergency drills and other documents were reviewed. The center is clean and organized with a large outside play area. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of nine children's records, the center did not obtain a child's physical within one month after attendance. Evidence: 1. Child B began at the center on 09/06/2016 and did not have a physical examination on file with the center.

Plan of Correction: I requested a copy of the physical from the parent and emailed the parent the form today.

Standard #: 22VAC40-185-160-A
Description: Based on review of nine staff records, the center did not obtain documentation of a negative tuberculosis screening from each staff member within 21 days after they were hired for employment. Evidence: 1. Staff C began working at the center on 4/03/2018 and did not have documentation of a negative tuberculosis screening.

Plan of Correction: I will ask the staff member to have the screening or get a note from the doctor.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. Pooh's Pals classroom had a disinfectant spray and disinfectant wipe in a high cabinet that was unlocked.

Plan of Correction: The teacher immediately moved the cleaning products under the sink in a locked cabinet.

Standard #: 22VAC40-185-280-H
Description: Based on observation, purses were not stored in areas inaccessible to children. Evidence: 1. One pink purse was hanging near Forest Friends classroom door on a hook and was accessible to children. The children were outside at the time.

Plan of Correction: The purse was moved to a locked closet.

Standard #: 22VAC40-185-500-A
Description: Based on observation, hand washing procedures were not followed in one classroom. Evidence: 1. One child in the Nemo's world class was observed finishing her snack and then sat down with the rest of the class in circle time. She did not wash her hands with soap and running water or disposable wipes after eating the snack.

Plan of Correction: The staff were reminded to make sure the children wash their hands after they finish snack.
