Comments:
This unannounced monitoring inspection was conducted from 10:15 am until 12:30 pm. At the time of the inspection 18 children were in care with three staff. The sample size consisted of five children's records and four staff's records. Children and staff were observed during outdoor and indoor free play, art activities, hand washing, snack, cleanup after snack, bathroom break, transitions, and behavioral management. Violations from the previous inspection were reviewed for current compliance. No repeat violations were found. Violations were found during this inspection and are identified on the violation notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, the administration failed to obtain documentation of physical examination prior to the child's first date of attendance or within one month after attendance for one out of five children's records. Evidence: 1. The record for child 3 has the first date of attendance documented as 8/30/17. The only physical in the record is dated 11/1/17. 2. An administrator verified the 11/1/17 physical is the only one in the file.

Plan of Correction: In the future physicals will be obtained within the first month of attendance.

Standard #: 22VAC40-185-510-J
Description: Based on observation, the center failed to ensure all medication is kept in a locked place using a safe locking method that prevents access by children. Evidence: In the staff restroom, located in the hall beside the children's restroom and across the hall from the classroom, on an open shelf above the toilet is a tube of Hydrocortizone Plus cream. Children go to the restroom unattended and the staff restroom door was open.

Plan of Correction: The cream will be moved to a locked location.

Standard #: 22VAC40-185-520-D
Description: Based on observation, the center failed to keep insect repellent in a location out of reach of children. Evidence: In the staff restroom, located in the hall beside the children's restroom and across the hall from the classroom, on an open shelf above the toilet is a spray can of Off insect repellent. Children go to the restroom unattended and the staff restroom door was open.

Plan of Correction: The insect repellent will be moved.
