Comments:
An unannounced monitoring inspection was conducted today from approximately 11:20am-3pm. There were 163 children in care directly supervised by 28 staff. A sample of 10 children's and staff records were reviewed. The children were observed eating lunch, during hand washing and bathroom procedures, and during dismissal. 3 new staff have been hired since the last inspection. School is interviewing for an additional new position. Areas of non-compliance are identified in this report. The exit interview was conducted with the HR Manager. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: 10 of 10 children's files were missing required information. 1) Child A-J were missing a nickname a nickname. 2) Child A was missing the addresses of 2 emergency contacts. 3) Child C was missing the employer and and employer phone number of both parents. 4) Child D was missing the employer phone number of both parents. 5) Child E was missing the mother's employer, employer phone number, phone number of one of 2 required emergency contacts and address of 1 of 2 required emergency contacts.

Plan of Correction: We will be updating forms for the upcoming school year. We will add required information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center were not maintained in a clean, safe and operable condition. Evidence: The swing poles on the playground were chipping black paint.

Plan of Correction: The playground was inspected two weeks ago and maintenance is planned for Saturday.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place. Evidence: Cleaning materials labeled "bleach water solution" were found in an unlocked cabinet in "Jamboree Room"

Plan of Correction: Staff will lock the cabinet.

Standard #: 22VAC40-185-290-3
Description: Based on observation, electrical outlets were not protected with protective covers. Evidence: 2 electrical outlets on a power strip connected to the fish tank located in the hallway across from the "Galaxy Room", were not protected with protective covers.

Plan of Correction: Outlets will be covered.
