Comments:
An unannounced monitoring inspection was conducted on May 18, 2018. Upon arrival to the facility, there were 11 children present with three staff members. The licensing inspector reviewed supervision, staff qualifications and trainings, nutrition, required posting, the physical plant and administration. The center is not administering medication at this time. The children were eating lunch and napping during the inspection. Exit interview conducted with the Program Director.

Description: Based on interview and documentation, the licensee did not ensure injury prevention procedures was updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: During inspection on May 18, 2018, the injury prevention plan was not updated annually.

Plan of Correction: The program director will develop updated injury prevention plan.

Standard #: 22VAC40-185-550-C
Description: Based on observation and inspection of the facility, the licensee did not ensure emergency evacuation and shelter-in-place procedures/maps were posted in a location conspicuous to staff and children on each floor of each building. Evidence: During the inspection on May 18, 2018, there was not posted procedures for shelter in place.

Plan of Correction: The program director will develop updated posted procedures for shelter place.
