An unannounced monitoring inspection was conducted today, May 21, 2018 to determine the center's compliance with licensing standards. The Inspector initiated the inspection at 9:45 am and concluded the inspection at 11:15 am. Upon the inspector's arrival there were 13 children in care with three staff. Children were gathered for circle time where they reviewed the day's of the week, months and counted to one hundred. Children also played a bean bag game where they gave the sound of the letter printed on the bean bag. Later children played in centers (blocks, tablets, painting at the easel, water table, and house keeping) while staff circulated around participating in play with the children. Systems were in place for maintaining supervision and ratio during bathroom breaks. The classroom has age and stage appropriate materials and supplies. The parent board had the required information posted to include the license, emergency numbers, evacuation maps/ procedures, menu, daily schedule and various parent information. The facility's first aid kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Three staff records and five children's records were reviewed. Staff have current CPR, first aid, daily health observation and MAT. Medications are administered by the elementary school nurse. Injury reports reviewed. The most recent evacuation drill was documented on 04/26/2018. The two shelter-in-place drills were completed on 09/28/2017 and 03/26/2018. Health Inspection 02/01/2018 Fire Inspection 05/11/2017 Today's lunch consisted of chilli and cheese hot dogs, baked beans, applesauce and milk. Description: Based on review of children's records, the facility did not obtain all required information for each child's record. Evidence: The record of child #1 did not have the name, address and phone number listed for a second emergency contact.

Plan of Correction: Per the staff: We will have the parent complete the missing information.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the facility did not ensure the finding of the most recent inspection of the facility were posted at the facility and in a place conspicuous to the public. Evidence: The inspection notice for the most recent inspection, conducted on 12/18/2017, was not posted.

Plan of Correction: Per staff: We will request a copy of the inspection notice from the administrative Head Start office.
