An unannounced monitoring inspection was conducted with the director beginning at 9:58 AM and ending at 2:30 PM. There were 95 children in care. The infants were observed playing with manipulatives, doing tummy time, and sleeping in cribs. The older children were observed playing with manipulatives, coloring, and playing outside on the playground. Children were also observed playing educational games on tablets. Ten children's records and five staff records were reviewed. Seven medications were reviewed and in compliance. The emergency drill log was reviewed and in compliance. The most recent emergency escape drill was documented as 4/30/18. The most recent shelter-in-place drill was documented as 5/15/18. The most recent lockdown drill was documented as 2/15/18. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent health inspection was documented as 11/29/17. Standard #: 22VAC40-185-80-A Description: Based on review of documentation, the center did not ensure all children were included on the written attendance record. Evidence: The attendance sheet in the Preschool I classroom had thirteen children marked present on the attendance sheet but there were twelve children present. The attendance sheet in the Preschool II-A classroom had twenty-one children marked present on the attendance sheet but there were nineteen children present. The attendance sheet in the Pre-K classroom had eighteen children marked present on the attendance sheet but there were nineteen children present. The attendance sheet in the Infant A classroom had ten children marked present on the attendance sheet but there were twelve children present. Plan of Correction: The center will retrain teachers on documenting children attendance on the written daily attendance sheet. The center will provide a transitional attendance sheet to document when children transition to different classrooms.
Standard #: 22VAC40-185-350-A Description: Based on observation and staff interview the center did not maintain required staff-to-child ratio when directly supervising children. Evidence: On Monday, May 21, 2018, there were thirteen children in the care of one staff member in the Preschool I classroom. Plan of Correction: Coverage was provided by management during the inspection. The center is in the process of hiring new staff to ensure the required staff-to-child ratio.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of documentation, the center did not obtain the central registry findings for all staff within 30 days of employment. Evidence: Staff A's record did not have the results of the central registry finding. Plan of Correction: The center has resubmitted the request for the central registry finding back to the Office of Background Investigation on May 8, 2018. The center will follow up on the status of the request.
